Cemetery Forms

In the event of passing away of a loved one in family please call Br. Maqbool at (Eight Zero One) 577-7901 and he should take care of the complete process. If someone in your family is very sick and a death is imminent then calling the number above ahead of time makes things easier for us. The steps should give you an idea how you should proceed.

1. Call and talk to Br. Maqbool.

2. Make an appointment with him to visit Khadeeja Cemetery for the purchase of a burial plot.

3. Bring personal information of the prospective owner of the burial site with you.

4. Complete the “Cemetery Form” and make payment of the cost.

5. Call Br. Maqbool as soon as the death occurs (see above for contact information).

You can skip Point # 2, 3 &4 if you are able to email the above Cemetery & Mortuary forms and make payment on the phone.
